[LV EV 3] Central Hepatectomy with Glissonean Pedicle Appraoch
Masakazu Yamamoto
Tokyo Woman’s Medical University, Japan

Couinaud described three main approaches to control the inflow system at the hepatic hilus in liver surgery; the intrafascial approach, the extrafascial and transfissual approach, and the extrafascial approach. The intrafascial approach is the so-called control method. The extrafascial and transfissual approach, and the extrafascial approach are recognized as the Glissonean pedicle approach. When the Glissonean pedicles are ligated before liver transection, various types of anatomical hepatectomy can be performed. Central hepatectomy of the liver is performed for patients with liver tumor in segment 4, 5 or 8. However, if we could not perform Glissonena pedicle approach, central hepatectomy is considered to be very difficult. We would like to show video clips of cental hepatectomy with the extrafascial and the extrafascial and transfissual Glissonean pedicle approach.
