[P138] Torsion of the wandering spleen : A case report

Introduction : Wandering spleen, also called ectopic spleen, is defined as movement of spleen from noraml location to another lesion. Absence of one or all of ligaments holding spleen, It usually loacted in the middle part of left abdomen. It is very rare disase, with less tahn 0.2% incidnce. It is commoly diagnosis in yound children. In this case, We will discribe 15 years old child who got splenectomy due to infarction of spleen.
Methods : Case
Results : A 15-year-old woman presented to the emergency department after three days of abdominal pain, vomiting and constipation without fever. She had a recurrent abdominal pain. On physical examination, abdominal distension was noted. There was a moderate diffuse abdominal tenderness more pronounced on the left side. Abdominal radiography showed an important stomach and small bowel distention especially at the upper left quadrant without air fluid levels . Computed tomography (CT) revealed a severe stomach and small bowel distention and large congested spleen was in the left lower quadrant. The patient underwent emergency hlaparotomy. Her spleen was in an ectopic position. It was rounded by a large and long small bowel and there was no sustentaculum lieni, nor any of the spleen ligaments. The spleen pedicle was twisted with significant ischemia. The patient recovery was uneventful. She was discharged on the sixth days.
