[P090] Carcinoma gall bladder: Is it worthy for surgery or not?

Introduction : Carcinoma Gall Bladder is the most common of extrahepatic biliary malignancy and fourth commonest upper gastrointestinal malignancy. It traditionally has been considered an incurable disease with and extremely poor prognosis. Because of the high frequency of advanced stage at presentation, long term survival is poor. Some studies show that documented 5-year survival rate is 5% and a median survival of 5 months. The aim of this retrospective study is to review all patients diagnosed as carcinoma Gall Bladder over a 4-year period to assess survival of patients.

Methods : Patients diagnosed as Carcinoma Gall Bladder, by histologically at post-operative period were studied retrospectively. The study period is 4 years (January 2014 to December 2017). Audit database and case-note review was performed.

Results : The 24 patients who were studied (Male :Female- 1:2) had a mean age of 60 years (ranges from 77 – 36 years). The operations underwent were Cholecystectomy in 7 patients (29 %), Radical cholecystectomy in 6 patients (25%), Cholecystectomy and liver resection in 7 patients (29%), Cholecystectomy and segmental resection of Transverse colon and stomach in 1 patient (4%), subtotal cholecystectomy (debulking tumour) in 1 patient (4%) and Biopsy (4%). The overall median survival was 7 months.

Conclusions : In this review of our case series, most of the Ca Gall Bladder patients are advanced stage at the time of presentation. Only four patients are alive and among them, only one exceeds one year survival. Even in radical resection, survival rate is not so much improving in management of Ca Gall Bladder.
