[BP BEST OP 2] Impact of the bile duct resection margin distance on survival of patients with resected extrahepatic bile duct cancer

Introduction : Prognostic relevance of the bile duct resection margin in extrahepatic bile duct (EHBD) cancer remains controversial. The aim of this study was to evaluate the survival outcomes according to the bile duct resection margin status and distance in patients with resected EHBD cancer.

Methods : A total of 308 patients who underwent potentially curative resections for EHBD cancer between September 2003 and June 2016 were enrolled in this retrospective single-center study.

Results : The status of the bile duct margin was classified as negative (R0) in in 235 patients, HGD (R1-HGD) in 38, and invasive carcinoma (R1-IC) in 35. There was no significant difference in disease-specific survival rates among 3 groups. When R0 and R1-HGD groups were divided into subgroups with a cutoff of 10-mm resection margin, the 5-year disease-specific survival rate of R1-HGD≥10mm (n=12, 88.9%) group was significantly higher than those of R1-HGD

Conclusions : The results of this study suggest that the distance of ductal resection margin is more important than the ductal margin status in predicting the survival of patients with resected EHBD cancer.
